Pharmacodynamics

Tetracycline is an antibiotic of wide effect spectrum.

Inhibits the synthesis of proteins in microbe cell. Hasbacteriostaticeffect. Active in most of Gram-positive and Gram-negative microorganisms, Spirochaeta, Leptospira, Rickettsia, serious virus (agents of Trahoma and Ornithosis), some protozoa (Trichomonas, Amoebae and others).

Pharmacokinetics

Tetracycline is absorbed easily (to 66% of taken dose). Penetrates into all organs and tissues, bile, synovial, ascetic, cerebrospinal fluids. Through undamaged hematoencephalic barrier penetrates badly, but the penetration into cerebrospinal fluid increases in inflammatory processes in meninx. With plasma protein interacts by 65%. Accumulates selectively in bones, teeth, liver, spleen, in quantity accumulates in tissues of tumors. Penetrates through placenta into breast milk. Excreted with urine and feces, partially with bile. Good resorption in digestive tract, but concentration in intestine is sufficient for treatment of intestinal infections.In case of disorder of kidney excretory function, cumulation is possible.

Indications

The drug is prescribed for the treatment of bronchitis, pneumonia, pleurisy, subacute bacterial endocarditis. Used in actinomycosis, gingivitis, stomatitis, otitis media, pharingitis, sinusitis, purulent meningitis, cholecystitis, angina, scarlatina, pertussis, bacterial and amebic dysentery, brucellosis, tularemia, epidemic typhus and relapsing fever psittacosis, gonorrhea, syphilis, in infectious diseases of genitourinary tracts, skin and tissues infections, and other infectious diseases caused by microorganisms that are sensitive to this antibiotic. Tetracycline may be used for the prevention of infectious complications in surgical patients.

Dosage and administration

The drug should be taken during a meal or at once after a meal. The dose for adults should be as 0,2—0,25 g three-four times daily (should be washed down). For children above 8 years the drug is used as 20—25 mg/kg daily.

Courseoftreatmentis 5—7 days. When symptoms of the treatment subside the drug should be still taken for 1—3 days.

In case of itch, heat, reddening of skin the treatment should be ceased.

Specialinstructions

Tetracycline should not be taken with milk or other dairy products simultaneously because of the bad absorption of antibiotic in this case.

Adverse reactions

PatientsusuallytolerateTetracyclinewell. There may be dyspeptic reactions, liver and kidney affection, pancreatitis, vertigo, photophobia, shaky manner of walking, pigmentation of skin, mucous membranes, enamel in a child, high intracranial pressure, dysbacteriosis, hypovitaminosis, photosensitization, allergic reactions.

In case of allergic reactions the drug should be canceled, the patient should undergo  desensitizing therapy. For the prevention of candidiasis progression nystatin or levorin is prescribed with Tetracycline. Simalteneous prescription of vitamin B during the treatment is reasonable.

Contraindications

Tetracycline should not be used in hypersensivity to it and such antibiotics, in fungous diseases, in expressive disorder of liver and kidney disorder, during pregnancy, lactation (breast-feeding is forbidden during the treatment), for children under 8 years.

Overdosage

Precautions

With care it is prescribed in leucopenia.

Druginteractions

Should not be used with the preparations that contain ions of metals (aluminium, calcium, magnesium, bismuth, manganum, zinc, preparations of iron) because of the formation of inactive chelates. Antimicrobic activity of Tetracycline  decreases in simalteneous use with barbiturates, carbamazepine owning to increase of metabolism of antibiotic through the induction of microsomal ferments of liver. Should not be prescribed with penicilins (the preparations have antagonistic effect on microorganisms). Tetracycline increases the concentrations of lithium, digoxin in plasma, increases effects of oral anticoagulants, may decrease effectiveness of oral contraceptives.
